How Long Is a DOT Medical Card Valid?
A standard DOT Medical Examiner’s Certificate (DOT medical card) is valid for up to 24 months. However, not every driver qualifies for a full 2-year card. Your certification period depends on your health at the time of your exam:
A full 2-year certificate is issued when all health markers meet FMCSA standards — blood pressure under 140/90, vision, hearing, no disqualifying conditions. A 1-year certificate may be issued for controlled high blood pressure (Stage 1 hypertension), treated sleep apnea with CPAP compliance, or well-controlled diabetes. A 3-month certificate may be issued for Stage 2 hypertension (160–179/100–109), giving the driver time to get blood pressure under control before a follow-up exam. A shorter certificate may also be issued for other conditions requiring monitoring.
When Should Ohio CDL Drivers Renew Their DOT Medical Card?
The FMCSA recommends scheduling your DOT physical renewal before your current card expires. Ideally, plan your renewal 30–60 days before the expiration date. This gives you a buffer in case you need to address a health issue, gather documentation, or follow up after a borderline blood pressure or other finding.
Ohio law aligns with federal FMCSA regulations — operating a CMV with an expired medical card is a violation that can result in fines and being placed out of service. Your carrier or employer may also have internal policies requiring renewal well in advance of expiration.
The DOT Medical Card Renewal Process
Renewing your DOT medical card is exactly the same process as your original exam. You’ll complete the FMCSA Medical Examination Report (Form MCSA-5875) and undergo the full physical evaluation including vision and hearing tests, urinalysis, blood pressure and vital signs, cardiovascular assessment, musculoskeletal exam, and review of your medical history since your last exam.

How Your DOT Medical Card Renewal Affects Your CDL
Your CDL is linked to your DOT medical card through your state’s DMV. When you renew your DOT physical, the results are electronically submitted to the FMCSA National Registry, which notifies your state DMV. In Ohio, the BMV updates your CDL record to reflect your new medical certification expiration date.
