For Ohio commercial truck drivers, few things are more stressful than wondering whether a health condition could cost you your CDL. The good news is that many conditions that once disqualified drivers are now manageable with proper treatment and documentation. But there are still certain situations that can result in disqualification or a shorter certification period — and knowing about them in advance can help you prepare.

Conditions That Can Disqualify a Driver from a DOT Physical

Uncontrolled High Blood Pressure

Blood pressure at or above 180/110 (Stage 3 hypertension) is a disqualifying condition. Drivers in Stage 1 or Stage 2 hypertension can often still be certified, but for shorter periods (1 year or 3 months) with a treatment plan. Get your blood pressure under control before your exam to maximize your certification period.

Untreated Obstructive Sleep Apnea

Untreated obstructive sleep apnea (OSA) is a disqualifying condition under FMCSA guidelines. However, drivers who are diagnosed and actively treating their sleep apnea with CPAP therapy — and can document compliance — are generally eligible for certification. Don’t ignore this condition. Get diagnosed and treated, then bring your compliance data to your exam.

Epilepsy and Seizure Disorders

A history of epilepsy or seizure disorders is generally disqualifying for commercial drivers under federal standards. However, drivers who have been seizure-free for a specified period without medication may qualify for an exemption in some cases. This requires careful documentation and review by the medical examiner.

Vision That Cannot Be Corrected to 20/40

If your vision cannot be corrected to at least 20/40 in each eye, you will not pass the vision requirement. Monocular drivers (vision in one eye only) are ineligible under the standard federal standard but may apply for a vision exemption through the FMCSA.

Insulin-Dependent Diabetes (Without Exemption)

Insulin-dependent diabetes is disqualifying without a federal exemption. The FMCSA has an exemption program for insulin-dependent diabetic drivers who meet strict criteria. Drivers with Type 2 diabetes managed by diet or oral medication are generally certifiable.

Substance Use and Positive Drug/Alcohol Tests

Use of Schedule I controlled substances (including marijuana under federal law) is disqualifying. Positive pre-employment or random drug tests, or a confirmed history of substance abuse without documented treatment, can prevent certification. The DOT physical urinalysis is a health screen, not a drug test — but your medical history form asks about drug use, and lying on it is a federal offense.

Certain Heart Conditions

Recent myocardial infarction (heart attack), angina, arrhythmias, and other serious cardiovascular conditions may be disqualifying without adequate recovery time and specialist documentation. Some drivers with cardiac history can be certified with appropriate documentation from their cardiologist.

What Does NOT Automatically Disqualify You

Many Ohio truck drivers worry unnecessarily about conditions that do not automatically prevent certification. Controlled high blood pressure with medication, treated sleep apnea with CPAP compliance, well-controlled Type 2 diabetes, most correctable vision problems, hearing loss correctable with hearing aids, and many mental health conditions treated with appropriate medication are all conditions that, with proper documentation and management, typically allow certification to proceed.
